In order to complete registration, it is necessary that you print out, complete and submit the following forms for your first visit to Malahat Disc Golf:
If you are unable to print the forms, you will be required to complete them at Malahat Disc Golf, prior to playing. Please note: Form completion/submission is only required upon your first visit to Malahat Disc Golf. This is not to register for the tournament. Call Dana at 250-480-6696.
